Position Overview

As a Licensed Practical Nurse (LPN), you'll provide direct and individualized nursing care to our clients under the supervision of the Director of Nursing and/or a Registered Nurse designee. The LPN plays a critical role in ensuring the safe and efficient delivery of home health services, supporting patients with self-care activities, and contributing to the coordination of our home health services.

Key Responsibilities

1. Provide direct and individualized nursing care to assigned patients, guided by nursing standards and under the supervision of the Director of Nursing and/or a Registered Nurse designee.

2. Implement the plan of care formulated by the Registered Nurses.

3. Ensure accurate and timely documentation in line with the patient's care plan.

4. Educate and provide information to patients and family caregivers pertinent to diagnosis and care plan.

5. Maintain a safe and healthy working environment, adhering to company clinical standards.

6. Participate in the coordination of home health services in collaboration with Registered Nurses and clinical management.

7. Assist patients in learning appropriate self-care activities.

8. Use medical equipment and supplies efficiently and effectively.

9. Document and report activities as required.

Skills/Qualifications

- Valid LPN license.

- Experience in home care or related field.

- Proficiency in providing direct patient care and implementing care plans.

- Excellent patient assessment and home health care delivery skills.

- Ability to educate patients and families.

- Ability to collaborate with other healthcare professionals.

- Knowledge of safe work practices in a home care setting.

- Proficient documentation skills.
