APN Healthcare Solutions is urgently hiring multiple Licensed Vocation Nurses LTC for our client in Mt. Angel, OR for a 13 week contract with a high possibility of extension.


Position Title: LVN/LPN LTC
Pay Rate:$40/43 hr
Work Location: Mount Angel, OR 97362
Shift : Evenings shift available
Hours: Mid 5x8-Hour (14:00 - 22:30)
Contract Duration:13 weeks
License Requirement: OR LVN/LPN
Certification Requirement: BLS

Job Description

Job Title: Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN)

Job Summary: As a Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) in our Long term care division, you will play a crucial role in delivering exceptional healthcare services to our clients. You will work closely with patients, their families, and a multidisciplinary team to develop and implement individualized care plans.

Key Responsibilities:

Assessment and Care Planning: Conduct comprehensive assessments of patients' health status.

Medication Management: Administer medications and treatments as prescribed by the physician. Monitor medication compliance and educate patients and families on proper administration.

Wound Care and Treatment: Provide skilled nursing care including wound dressings, injections, and other treatments as required.

Monitoring and Documentation: Regularly monitor vital signs and document changes in patients' condition.
Maintain accurate and up-to-date patient records in accordance with regulatory guidelines.

Education and Support: Educate patients and their families on disease management, self-care techniques, and preventive measures.
Provide emotional support and counseling to patients and their families.

Collaboration and Communication: Collaborate with physicians, therapists, and other healthcare professionals to coordinate and optimize patient care.
Communicate effectively with the healthcare team to ensure continuity of care.

Adherence to Regulations: Adhere to all relevant federal, state, and local regulations, as well as agency policies and procedures.
