Legends Healthcare is a premier long term care company operating twenty one long term care facilities throughout the state of Ohio and is now hiring Full-Time and Part-time (LPN) Licensed Practical Nurses at Cardinal Woods Skilled Nursing & Rehab, located at 6831 Chapel Rd, Madison, Ohio.

We are currently seeking skilled, compassionate, dedicated, and driven Licensed Practical Nurses (LPN) to welcome our ever-growing team.

Job Duties and Requirements:
- Valid Ohio State License(s)
- Reliable Transportation
- Current license as a Licensed Practical Nurse in the State of Ohio required.
- Is a graduate of an accredited school of Licensed Practical Nursing.
- Successfully completes facility conducted orientation, mandatory training and in-service programs.
- Previous long term care experience is required. Supervisory experience and organizational skills in a long term care setting preferred.
- Must meet health assessment requirements, including two stage Mantoux skin test.
- Is listed in good standing as confirmed on the Ohio State Department of Professional Credentialing.
- Possesses excellent technical assessment and documentation skills, and leadership quantities.
- Must be capable of performing the essential functions of the job, with or without reasonable accommodations.
- Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required
