Licensed Practical Nurse (LPN) Registered Nurse (RN) Reports To: Director of Nursing

Job summary for Licensed Practical Nurse (LPN) Registered Nurse (RN):

Licensed Practical Nurse (LPN) Registered Nurse (RN) is responsible for providing and coordinating nursing care in compliance with facility policies and procedures and to assess residents' care needs and take appropriate action. As well as responsible for the clinical direction and management of the STNA duties within the assigned nursing unit.

Responsibilities for Licensed Practical Nurse (LPN) Registered Nurse (RN):

1. Ensure accurate and timely administration and distribution of all medications to assigned residents.

2. Administer and document direct resident care, medications and treatments per physicians' orders and accurately record all care provided. Follow established standards of nursing practices and implement facility policies and procedures.

3. Obtain report from and provide report to nurse being relieved or coming on duty and record/relay sufficient information to implement appropriate follow-up action as necessary.

4. Collect data and properly document comprehensive assessment of residents as assigned.

5. Develop and implement an accurate comprehensive care plan based on each resident's needs and his/her comprehensive assessment.

6. Identify resident problems and emergency situation and initiate emergency care and "life-saving" measures in the absence of a physician.

7. Provide supervision to Nursing Assistant providing direct resident care.

8. Counsel Nursing Assistants and recommend disciplinary action to the Director of Nursing Service, when necessary.

9. Promptly report changes in residents' conditions to the physician, Director of Nursing Service and responsible party, and take follow-up action as necessary.

10. Order, receive and store medications appropriately.

11. Document resident progress notes as required.

12. Consistently work cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies and ancillary service providers.

13. Obtain continuing education required to maintain licensure.

14. Adhere to all infection control policies within the assigned facility and observe infection control and safety procedures.

15. Attend and participate in all scheduled staff meetings, educational meetings/in-services and all other meetings related to employment.

16. Assist with and participate in all resident activities to facilitate a home like environment for the residents within assigned facility.

17. Ensure compliance with all HIPAA regulations.

18. Respectfully engage and welcome all visitors, guests and family members within the facility.

19. Ensure a safe and sanitary living environment for residents.

20. Ensure the Resident Assessment Instrument (RAI) process is followed in accordance to State & Federal Guidelines.

21. Follow Residents' Rights policies at all times.

22. Adhere to all facility policies & procedures.

Position Qualifications for Licensed Practical Nurse (LPN) Registered Nurse (RN):

EDUCATION AND EXPERIENCE:

Graduate of an accredited Nursing Program.

Valid License to practice as LPN or RN in the State of Ohio.

CPR certification required.

1 year of nursing experience preferred.

Ability to add and subtract and perform basic mathematics as it relates to the position is required.

Ability to read/write/comprehend the English language in written & verbal form required.

Ability to communicate both verbally and non-verbally required.
