Position Details Expiration Date 5/31/2024 Department MC Fam Med Rural & Comm Med Position Type Civil Service Location Alexandria Description Incumbent is assigned to the clinic area in the Family Practice Clinic, and provides basic skilled nursing care for convalescent, chronic and acutely ill patients in stable and predictable situations in accordance with the Professional Nurses Practice Act and under the direct supervision of the RN Supervisor. Primary duties as the Community Care Coordinator are daily reviews of the Medicaid web site and pulling the roster for the clinic at LSU Family Practice Medicine in Alexandria, Louisiana. Assessment of children needing appointments in clinic and letters will be sent to the parents of said children. The Community Care Coordinator makes necessary updates to the LINKS system, including insuring that all vaccines are correctly recorded once the child has been seen in the clinic. If the documented information in LINKS is not correct, she must submit it to the state auditor to be corrected and then follow up to assure it has been corrected. The Community Care Coordinator must also check the Medicaid web site for billing updates. The Community Care Coordinator must view Residents and/or Faculty's chart notes as well as the nurse's notes for proper documentation and billing of ordered vaccines. If errors are noted, the Community Care Coordinator is to notify billing, the Resident and/or Faculty and the nurse responsible for the error/errors. The Community Care Coordinator orders VFC vaccines, manages and/or locates vaccines when her inventory is not correctly accounted for. She/he is responsible for educating staff on any vaccine changes. Responsible for refrigerator Quality Assurance and if the electricity fails, she must inventory and transport any vaccine to and from the hospital. The Community Care Coordinator is also responsible for up to 6 audits per year. In addition to the above duties, if a nurse calls in or takes vacation, the Community Care Coordinator is expected to take a team to assist with clinic patient care. In addition to the above duties, the Community Care Coordinator is expected to perform related patient care services needed to meet the health care needs of outpatients in accordance with the medical plan of care, the nursing plan of care and the clinic standards of care. He/she must demonstrate growth and development knowledge and skills by providing nursing care appropriate to infants, children, adolescents, adults and geriatrics that present them in the clinic. He/she must utilize observational skills and report pertinent patient symptoms and changes in condition to a professional nurse or medical staff as well as assist with emergency situations. Documents observations and nursing care implemented such as medications, treatments, and patient education. Assist with management duties such as maintaining adequate supplies and maintaining a clean organized and safe environment. Participate in the continuing Quality Improvement activities of the clinic to assure of the clinic continuous quality improvement. Work independently under the general supervision of a Registered Nurse. Completed work is reviewed for adequacy in terms of orientation, checklists, university/nursing policies and procedures, federal/state laws, and LSUHSC-S nursing standards of care. LPN 3 may provide functional supervision over LPN 1's and LPN 2's. Typical work hours are Monday through Friday from 8:00 AM to 5:00 PM or until work is completed. Clinic census may cause hours to be extended in order to complete care, treatment, etc. Lunch and break times are dictated by workflow and taken as assigned by the Supervisor. Work hours may change as department needs change. Qualifications MINIMUM QUALIFICATIONS: Possession of a valid Louisiana license or temporary permit to practice practical nursing or a multi-state license issued by a Nursing Licensure Compact (NLC/eNLC) state.
