Qualifications:
  • High School diploma required. Bachelor's degree in Psychology, Social Work or related field preferred. Experience in case management, advocacy, outreach, assessment, crisis intervention and supportive counseling necessary as well as strong ability to communicate clearly.
  • Valid Driver's License

The JOBS Case Manager will provide direct outreach and support services to youth and young adults, allowing easy access, timely response, and flexibility to deliver services in the community with opportunities to grow toward optimally independent functioning in all areas of their lives.


Major Responsibilities


COORDINATION OF SERVICES: The JOBS Case Manager will Provide individual support to clients. Provide collaboration with services and community members. Assess client's, families, and community's strengths and needs to better facilitate services.


DIRECT SERVICE: The JOBS Case Manager Provides individual support and case management to support youth and young adults in developing skills to find gainful employment. The JOBS case manager will work with youth to gain independent living skills including areas of employment, finance, education, daily living tasks, and relationships. Assists in defining job goals, learning job tasks, and problem-solving various barriers to employment. Serves as liaison between the individual and work site when necessary. Helps individual increase skills in all areas related to employment. Provides case management services to individuals including assessment, advocacy, and monitoring. Facilitates community integration. Assists individuals in the development of other life skills related to independent adult living. Coordinates the intake and orientation of new clients to the Jobs Program. Works with youth in need of shelter to find a safe supported housing situation including entrance into either short term or long-term housing. Participate in the planning and implementation of division specific summer camps/services. Attend any school meetings, family safety planning meetings, CPS, or other meetings deemed necessary.


PROGRAM DEVELOPMENT & COMMUNITY AWARENESS: The JOBS Case Manager will Provide community awareness of programs and services provided by the JOBS program. Acts as liaison with statewide and community agencies. Facilitates networking with other providers to enhance emergency resources. Attends community meetings when appropriate to help develop relationships with possible referral sources and/or employers. Facilitate meetings between JOBS clients and HireAbility to ensure that all clients meet JOBS criteria and have a plan through HireAbility.The JOBS case manager should reach out to all new JOBS referral within 5 business days to explain the program and explain the intake process, if it is an external referral.


SUPERVISION AND TRAINING: The JOBS Case Manager Assists in facilitation of all treatment team meetings to promote effective communication; develops goals following identification of client's and oversees implementation of individual treatment plan. Maintain interagency, professional and parent/guardian communications, as well as communication between treatment team members to assure effectiveness of services. Attends workshops, conventions, meetings, etc., as agreed upon with supervisor.


ADMINISTRATIVE: The JOBS Case Manager Meets weekly with supervisor. Attend monthly meetings with HireAbility and Quarterly meetings with the State JOBS team. Help to complete and send in all Quarterly reports. The JOBS case manager is responsible for all required written documentation of case management services. Follow UCS recordkeeping requirements for all cases. Completes time sheets and other paperwork in a timely manner. All documentation should adhere to Medicaid standards.
