Integrational Clinical Lead

Intergration Clinical Lead Position Status : 0.1 FTE (4 hrs. per week) Stipend : $500.00 per month Internal providers are you interested in taking additional duties as an Intergration Clinical Lead. In this position you work your current provider position plus the additional duties listed below: SPECIFIC DUTIES: + Provide feedback on workflows to ease access more seamlessly for our clients. The work of integration between co-located departments is an ongoing process. In employing a clinician to provide their unique perspective on inter-departmental activity we not only recognize the complexity of bridging services throughout WSC but also commit to improvement + Collaboration: Dental, Mental Health, Psychiatry staff. Complex care manager. Possibly street outreach staff DESPCRIPTION : + Champion and lead integration strategies between WSC Dental and Medical. This has been a longstanding priority for NWHS and successfully modeled in FQHCs both within and outside Oregon. Conduct a review of dental-medical integration examples and summarize best practices to consider for adoption in NWHS. + Assume responsibility for maintaining integration between WSC Psychiatry / Mental Health and Medical. Attend the once MH staff meeting on the 3rd Tuesday each month (currently attended by associate medical director). + Work toward achieving Level 6 of the Collaborative model between behavioral health and medical care. + Collaborate with Complex care manager to enhance the quality and scope of community health worker and behaviorist contributions to patient care. + Assist with chart review by providing weekly feedback to physician assistants. + Mentor new hire clinicians during onboarding period. + Reports to medical director for semi-monthly standing meeting.
