About the Role:

Job Summary

Seeks out and manages referrals to the inpatient rehabilitation program. Conducts detailed pre-admission assessments in collaboration with the medical director in order to determine medical necessity for admission to The Inpatient Rehabilitation Program. Makes appropriate referrals outside our system when necessary. Assists in the advancement of the professional practice environment by communicating the NGHS nursing strategic direction and focusing on activities that support the nursing strategic direction. Provides nursing involvement support for VOICE and the NDNQI quality teams including coaching team members in their professional participation. Assists with overall clinical governance and other projects as assigned.



Minimum Job Qualifications

  • Licensure or other certifications: Licensed to practice as an RN in Georgia.

  • Educational Requirements: Associates Degree.

  • Minimum Experience: Two (2) years of experience.

Preferred Job Qualifications

  • Preferred Licensure or other certifications: Certified Rehabilitation Nurse.

  • Preferred Educational Requirements:

  • Preferred Experience: Three (3) years of med/surg or critical care nursing in a hospital setting and/ or one (1) year experience as an intake coordinator in an Inpatient. Rehabilitation setting.

  • Other: Professional organization membership within three (3) months of hire into the role, if applicable.

Essential Tasks and Responsibilities

  • Accepts referrals for rehabilitation assessment from any source (i.e., other health care facilities, physicians, family members, patients, etc.)

  • Ensures that appropriate authorization for an assessment is obtained based on referral source.

  • Performs an on-site evaluation as needed to investigate clinical course of patient to establish appropriateness for the program.

  • Performs telephone and facsimile evaluations for patients

  • Discusses with patient and/or significant other: Positive discharge plan, family participation in conferences and teaching sessions, ability to provide assistance for patient post-discharge if necessary, visiting hours, daily treatment/activity schedules, and policies and procedures.

  • Consults NGMC case manager, physiatrists, and other required health care professionals (i.e. dialysis unit, rehabilitation team members, etc.) to determine ability to meet needs/appropriateness for services.

  • Consults financial counselor to confirm insurance benefits and requirements.

  • If a clinical case is determined to likely meet medical necessity for admission to the inpatient rehabilitation program, the medical director is consulted in order to make official medical necessity determination.

  • Refers patients who do not meet admission criteria to other appropriate services provided by NGHS or other facilities, agencies, or community resources. Documents recommendations on assessment.

  • Ensures that the patient and/or significant other(s) receives appropriate written information and telephone numbers prior to admission.

  • Coordinates the date and time of admission with the referral source, the patient and/or significant others, and the receiving program secretary.

  • Ensures that admission information is completed and made available to program personnel prior to admission.

  • Ensures that appropriate medical records are sent with the patient or received prior to admission.

  • Obtains precertification when required and relays appropriate insurance information to case management.

  • Annually assists in developing and implementing a marketing plan with the program director.

  • Successfully completes mentoring class. Assists with mentoring of Registered Nurses when other mentor resources have been assigned. May mentor more as desired.
