As a Healthcare Liaison, this role combines clinical expertise with an in-depth understanding of patient needs post-discharge, aiming to facilitate smooth transitions and foster collaborative relationships with healthcare facility partners.
Key Responsibilities:
- Perform patient assessments, collaborating closely with healthcare teams to plan for post-discharge needs.
- Serve as a pivotal communication bridge between Stella Healthcare and healthcare facility staff, ensuring clear, coordinated care plans.
- Utilize expertise in care management to identify and arrange necessary patient support services post-discharge.
- Build and maintain professional relationships with healthcare facility partners, leveraging experience in care management to enhance service delivery.
- Advocate for patient needs, drawing on a deep understanding of the care requirements following discharge from a healthcare facility.
Qualifications:
- Valid LPN or RN license in New Jersey, with a minimum of 3 years of experience in a nursing facility or related healthcare setting.
