Job Overview: Vitability Health New Jersey is seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) with specialized experience in care management within nursing facilities. As a Healthcare Liaison, this role combines clinical expertise with an in-depth understanding of patient needs post-discharge, aiming to facilitate smooth transitions and foster collaborative relationships with healthcare facility partners.

Key Responsibilities:
  • Perform patient assessments, collaborating closely with healthcare teams to plan for post-discharge needs.
  • Serve as a pivotal communication bridge between Stella Healthcare and healthcare facility staff, ensuring clear, coordinated care plans.
  • Utilize expertise in care management to identify and arrange necessary patient support services post-discharge.
  • Build and maintain professional relationships with healthcare facility partners, leveraging experience in care management to enhance service delivery.
  • Advocate for patient needs, drawing on a deep understanding of the care requirements following discharge from a healthcare facility.
Qualifications:
  • Valid LPN or RN license in New Jersey, with a minimum of 3 years of experience in a nursing facility or related healthcare setting.
  • Proven experience in care management, specifically related to coordinating patient care post-discharge.
  • Strong clinical skills with the ability to assess patient needs accurately and develop effective care plans.
  • Excellent communication and relationship-building skills, capable of fostering trust and cooperation among healthcare teams.
