POSITION SUMMARY Relocation Assistance available up to $15,000 Pay Range: $64.06 to $96.09 Position Summary - The Infusion/Oncology Pharmacist is responsible for developing and directing all outpatient infusion and oncology pharmacy services. This position assumes tasks necessary to ensure the delivery of IV admixtures products and clinical monitoring services to the outpatient infusion and oncology patients. KEY RESPONSIBILITIES - Performed majority of the time: Responsible for clinical pharmacy oncology and outpatient infusion services Verifies orders and maintains patient profiles Performs critical dosing calculations for all patients of all age groups Documents and amends patient records in the electronic health record according to policy Works with the outpatient infusion services and oncology coordinators to ensure and maintain efficient customer service, resolve issues, and address concerns Able to incorporate current standards of practice into outpatient infusion and oncology practices Proactively reviews future patient schedules to ensure appropriate dosing and future delivery of medication Provides clinical drug information to other members of the healthcare team Able to identify potential issues related to availability of medication for infusion including ordering, insurance authorization, and regulations/policies Establishes and maintains guidelines, departmental policies and procedures, and collaborative practice agreements for infusion and oncology medications Safely work with hazardous drugs and be compliant with USP 800 Demonstrates knowledge of sterile technique and USP 797 Demonstrates knowledge of infusion devices and clinical considerations that apply Monitor departmental technology use related to ambulatory pumps. Evaluate and coordinate usage. Participates in reviews of equipment acquisition Supervises support personnel: educates and checks technician work Accurately dispenses medication in a timely fashion Understand the special considerations in compounding and stability related to the use of infusion and oncology therapies Participates in extemporaneous compounding, including sterile medications and chemotherapy Performed occasionally but critical to successful performance of the job: Orders medication and supplies from wholesaler Participates and delivers hospital educational programs Performs additional duties in the absence of the Pharmacy Director Participates in medication utilization evaluations and quality improvement projects Maintains adequate and updated resources related to outpatient infusion and oncology medications Decision making and budget responsibilities (e.g. impact limited to the employee, entire unit or organization-wide): Impact limited to the employee and care of individual patients No budget responsibility Assigned direct reports (not limited to the ones listed). N/A JOB SPECIFICATIONS JOB SPECIFICATIONS Education: Required Licensed as a registered Pharmacist in the State of Oregon Preferred Home infusion therapy certification Experience: Required Minimum three (3) years of hospital pharmacist experience including IV admixture and chemotherapy preparation Preferred Completion of a PGY-2 pharmacy oncology residency or eligible to take the board-certified oncology pharmacy (BCOP) examination Experience with outpatient infusion Licenses, Certifications and/or Registrations: Required Licensed as a registered Pharmacist in the State of Oregon BLS required within 90-days of hire ACLS required within 90-days of hire Preferred N/A Job Related Skills, Abilities and Behaviors: Required Excellent customer service skills Reads, writes and speaks conversational English Deals with multiple demands in a fast-paced pharmacy in an accurate efficient, calm, and pleasant manner Pharmacy computer skills desired, must be proficient at typing Presents a professional appearance and appropriate interpersonal skills Communicates effectively in verbal and written form with the healthcare team and with patients Functions as a cooperative, flexible team member in a positive manner Consistently utilizes problem-solving techniques to resolve complaints or concerns Demonstrates knowledge of sterile compounding technique Must be able to work independently with no direct supervision, set priorities, make critical decisions, respond quickly to needs, and adapt to changes in workload Preferred Bilingual skills a plus Previous experience with the EPIC electronic medical record, including Beacon, and Omnicell Additional Posting Information Hillsboro Medical Center believes in providing equal employment opportunities for all qualified individuals.
