Position Summary : The Hospice Visit Nurse will perform patient visits between Fridays 8 pm to 8 am and 8 am and 8 pm, one weekend per month . This position will work in collaboration with triage nursing to make skilled nursing visits to patients/families needing after hours support.

Job Requirements:
  • Displays an attitude of courtesy and respect for all prospective patients, families, staff and referral sources
  • Works closely with triage nurse and makes face to face visits as directed
  • Functions as a team member with other staff in the performance of duties.
  • Communicate significant findings, problems, patient's needs, reports changes in condition or environment to the physician, clinical manager and other members of the hospice care team and obtains physician orders as required
  • Seeks and accepts feedback regarding the performance of job duties.
  • Is aware of patient confidentiality and adheres to HIPAA regulations.
  • Displays an understanding of vulnerable adult issues.
  • Displays an understanding of hospice patients' rights.
  • Exhibits sound mature judgment.
  • Excellent Patient Care Delivery
Qualifications:
  • Graduate of accredited school of nursing
  • Current state of Minnesota RN or LPN licensure
  • Three years recent nursing experience in acute, long-term care or home care/hospice/public health
  • Patient care planning and patient teaching skills
  • Written and verbal communication skills
  • Critical thinker
  • Current Minnesota driver's license, insurance and daily access to reliable vehicle
  • Computer literacy
