Details:
The Registered Nurse (RN) will be responsible for providing care for patients at home or reside in a assisted living, personal care or skilled nursing facility. This position will be working seven days/nights on and seven nights/days off. Monday thru Thursday 12p-1am, Friday from 5pm until Monday 830 am.

Qualifications:
- Current RN or LPN licensure in state of PA
- Admissions and On-call .experience preferred
- Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and HHA leadership.
- Can work independently and on your own on a day-to-day basis with activities that include: phone, email, and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.
- Time management skills to manage visits, documentation, and meetings (i.e.:schedule visits, attend required meetings, and respond to urgent needs by prioritizing and triaging multiple concerns)
- Ability to learn an electronic medical records system.
- Must be willing to drive with reliable transportation, a valid driver's license, and auto insurance, to be able to travel across the service area
- Excellent organizational and time management skills including timely charting.
- Committed to safety precautions and high ethical standards
- Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplied and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills
