Job Summary: Provides skilled nursing care ordered by the physician as indicated by the plan of care to all patients on caseload. Review caseload with supervising RN on a regular basis as established by best practice and/or State guidelines. Obtains necessary physician orders and collaborates with the physician as the patient's needs dictate. Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care. Participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members (e.g. hospice aide, RN, OT, PT, MSW, ST, Dietitian, Chaplain or Hospice Manager) Uses equipment and supplies effectively and efficiently. Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications. Actively participates in Quality management program. Completes accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion. Education/Experience Requirements: Graduate of an accredited nursing program. Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in state of practice. Experience as an LPN/LVN commensurate with one of the following preferred: One year experience in an acute care setting within the last 24 months; Six months home care experience within the last 12 months preferred.
