The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
• Provides direct patient care as defined in State Nurse Practice Act
• Implements current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care
• Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group
• Uses equipment and supplies effectively and efficiently; assist in providing arrangement for equipment and other necessary items and services
• Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications
• Participates in personal, professional growth and development
• Participates in quality assessment performance improvement teams and activities
• Assists the patient with the activities of daily living and facilitate the patient's efforts towards self-sufficient and optimal comfort care
• Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required
• Provides and maintains a safe environment for the patient
• Assists the patient and family/caregiver and other team members in providing continuity of care
• Attends interdisciplinary group meetings
• Assures proper documentation of Nursing Services in clinical record in compliance with state and federal standards
POSITION QUALIFICATIONS
• Graduate of an accredited practical nursing program
• Two years nursing experience preferred. Community health/hospice or medical/surgical
• Currently licensed as a Licensed Practical Nurse in the state of Georgia
• Maintains a current CPR Certification
• Understands Hospice Philosophy and issues of death and dying
• Ability to deal tactfully with customers and the community
• Knowledge of business management and governmental regulations
• Demonstrates good communications and public relations skills
• Has excellent observation, verbal, and written communication skills
• Ability to work a flexible schedule
• Complies with Accepted Standards of Practice as required by job title/position
• Ability to operate computer equipment, fax, and copier machine
• Ability to assess and respond to the needs of the patients and families in varied settings
• Understands hospice care and the services provided to patient and family/caregiver through an interdisciplinary group
