JOB SUMMARY Provides personal care services and support within the guidelines of the Interdisciplinary Team Care Plan to terminally ill patients in their homes, in Skilled Nursing facilities or in Assisted Care facilities under the direction of an RN Case Manager. Assists and instructs the caregivers to provide personal care to the terminally ill patient. Provides emotional support and compassionate care to the patient and caregivers. Non-Exempt position. ESSENTIAL FUNCTIONS Performs all duties within the Certified Nurse Aide scope of practice. Provides palliative nursing care and ongoing support in the home as ordered on the plan of treatment and nursing care plan. Functions as a team member demonstrating collaboration with and responsibility to patient, physician and interdisciplinary team. Performs patient care responsibilities considering needs to the standard of care for patient's age. Provides personal care and assist with activities of daily living to terminally ill patients Performs prescribed activities per Plan of care including ambulation, transfers, and active/passive range of motion and ability to use assistive devices and durable medical equipment. Participates in Hospice's other mandated activities as requested by the Administrator including in-services and UR/PI committee. Communicates with RN care managers after patient visits. POSITION REQUIREMENTS Minimum Education High School graduate or equivalent Current Nurse Aide certification in the State of Texas required. Current CPR certification required. One year Nurse Aide experience required. One year prior hospice experience is preferred. Valid state driver's license, current automobile insurance and reliable automobile required. Required Skills Must possess superior ability to communicate effectively in English, both verbally and in writing. Additional languages preferred. Excellent interpersonal skills and customer service. Must possess high degree of organization and be able to multi-task efficiently and effectively. Basic computer knowledge required. Ability to use standard office equipment (copier, fax, etc.). FUNCTIONAL DEMANDS Functional Job Requirements: Job requirements include, but are not limited to: sitting, standing, walking, kneeling, bending, squatting and reaching. Requires good finger dexterity, repetitive motions with hands and fingers. Requires ability to have good vision and hearing. Pushing and pulling equipment may be necessary. Lifting Requirements Job requires heavy lifting on an occasional basis.
