Home Health Team Leader

Westminster-Canterbury on Chesapeake Bay is currently hiring a Home Health Team Leader, RN. Westminster-Canterbury At Home (WCAH) offers personalized and exceptional Home Health Care, Home Care, and Hospice services in private home settings in Virginia Beach and surrounding areas. Schedule: Monday through Friday, 8am to 5 pm (with monthly or bi-monthly on-call responsibilities). What will I do? The Home Health Team Leader, RN is responsible for providing initial and on-going assessment and care planning for home health patients and caregivers. Supervise and coordinate the implementation of home health orders for all home health clients. Provide assistance and training to clinicians to promote full understanding of their responsibilities related to documentation and clinical decision-making. Provide ongoing quality oversight of delivery of home health care. Supervise, communicate, and collaborate with other members of the home health team. What do I need for this role? Graduate of an accredited institution of professional nursing. At least two (2) years of clinical or supervisory experience in home health or related health care is required. Knowledgeable of applicable local, state, federal regulations. Holds an unencumbered state license as Registered Nurse. Current CPR Certification.
