The Home Health Social Worker role is to assist and counsel patients and families with financial, social, and emotional concerns in a Home Health setting. this role, Social Worker will and their families with personal and environmental difficulties that could with obtaining maximum benefits from health care. The position will also be responsible for coordination of discharge for transitioning to home care. Our Home Health Workers are required to: Have a Master degree in Social Work from an accredited college and a minimum of 1 year of Social Work in a health care setting; Be licensed as a Social Worker in the state of Pennsylvania or meet all education and experience requirements for licensure; Possess a valid driver license and maintain reliable transportation that is insured in accordance with Pennsylvania requirements; Maintain current CPR certification; Be able to effectively interact with residents, resident families, and team members; Have working knowledge of Internet Software, Spreadsheet Software, and Word Processing Software.
