Job Title: Home Health Registered Nurse
Location: Santa Rosa, CA
We are currently seeking an experienced Home Health Registered Nurse to join our team on a 13-week renewable contract basis. In this role, you will be responsible for providing exceptional patient care in the comfort of the patient's home.
Responsibilities:
- Provide high-quality nursing care to patients in their homes
- Perform assessments, develop care plans, and coordinate with other healthcare professionals to ensure optimal patient care
- Administer medications and treatments as ordered by physicians
- Educate patients and their families on disease management, medication regimens, and self-care techniques
- Document patient care and other relevant information accurately and timely
Requirements:
- Current RN license in California
- Minimum of 1 year of Home Health nursing experience
- BLS certification
- Excellent communication and interpersonal skills
- Ability to work independently and manage a patient load efficiently
