Providence at Home is seeking a Home Health Registered Nurse (RN), Full Time in Lansing, IL and various zoned locations throughout the south suburbs. Schedule Available: Monday - Friday during normal business hours. On-call after hours for incoming phone calls and triaging. Provides a complete physical assessment and history of current and previous illness(s). Regularly re-evaluates patient nursing needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Requirements Must be a Licensed Registered Nurse (RN) for the State of IL. Minimum of 1-2 years' experience, at least one of which in the area of home care nursing, hospice, ER or Med surg. Must be CPR certified. EOE
