Job Title: Home Health LVN Case Manager


Position Overview:

We are seeking a dedicated and experienced Licensed Vocational Nurse (LVN) to join our team as a Home Health LVN Case Manager. In this role, you will be responsible for coordinating and managing the care of patients in a home health setting. The ideal candidate will have excellent clinical skills, strong communication abilities, and a compassionate approach to patient care.


Responsibilities:

1. Patient Assessment and Care Planning:

- Conduct comprehensive assessments of patients' health status.

- Develop individualized care plans based on assessment findings, in collaboration with the interdisciplinary team.


2. Coordination of Care:

- Coordinate and manage the overall care of assigned patients.

- Collaborate with physicians, therapists, and other healthcare professionals to ensure a seamless and integrated care experience.

- Monitor and adjust care plans as needed to meet the evolving needs of patients.


3. Patient Education:

- Educate patients and their families on the management of health conditions, medications, and treatment plans.

- Provide guidance on preventive measures and wellness strategies.


4. Clinical Documentation:

- Maintain accurate and up-to-date patient records, documenting assessments, care plans, interventions, and outcomes.

- Ensure compliance with regulatory standards and agency policies.


5. Communication:

- Communicate effectively with patients, families, and the interdisciplinary team.

- Provide timely and relevant updates to physicians and other healthcare professionals involved in patient care.


6. Monitoring and Evaluation:

- Regularly assess patient progress and adjust care plans accordingly.

- Participate in case conferences and team meetings to review patient outcomes and address any challenges.


7. Quality Improvement:

- Participate in quality improvement initiatives to enhance the delivery of home health services.

- Contribute to the development and implementation of best practices.


Qualifications:

- Current and valid LVN license in the state of practice.

- Minimum of X years of experience in home health or a related healthcare setting.

- Strong clinical assessment and critical thinking skills.

- Excellent communication and interpersonal skills.

- Ability to work independently and as part of a collaborative team.

- Knowledge of Medicare/Medicaid regulations and home health accreditation standards.
