Location:Westlinks -12801 Westlinks DriveFort Myers FL 33913 Department: Quality Improve - Education Work Type: Full Time Shift: Shift 1/8:00:00 AM to 4:30:00 PM Minimum to Midpoint Pay Rate:$30.27 - $40.86 / hour Summary Office based position under direction of the Director Continuum of Care Business Operations. Responsible for analysis of SOC, Recertification and Resumption (and Other Follow-up OASIS) OASIS and accompanying documentation, management of proper utilization of resources to ensure positive patient outcomes and optimal financial outcomes. Teamwork, flexibility, and professionalism, while important in all functions of the System, are central to the success of the Home Health Team, and are expected in this role. Duties will include covering team member PTO, and other requested office tasks. Requirements Education: Associate degree in nursing required. Bachelors degree preferred. Bachelors degree in physical therapy required. Masters degree preferred. Experience: 2 Years of Case Management preferred. 2 Years of Home Health case manager experience preferred. Additional Requirements: Qualified individuals will successfully complete the competency assessment. Certifications: OASIS Certification required. Home Health Coding Certification required. License: Registered Nurse or Physical Therapist License in the State of Florida required.
