ROLE OVERVIEW and PURPOSE The Home Care Administrative Coordinator will coordinate authorized home care and home health services for Program for All-Inclusive Care for the Elderly (PACE) participants. This role will work with care teams to ensure proper care services are provided through contracted agencies to meet the needs of participants. In addition, this role will serve as the liaison between PACE and contracted home care and home health agencies. RESPONSIBILITIES Serves as main point of contact for contracted home care and home health agencies regarding referrals, authorizations, scheduling, and follow-ups Prepares, processes, and completes participants referrals for services, including communicating referrals, instructions, and schedule details to participants and their families Documents and tracks actions, services, and status updates in designated logs and participant electronic medical records (EMR) in accordance with clinical, DHCS, and CMS regulations and guidelines Monitors and reports status of authorization requests for participants; escalates issues to direct supervisor, as needed Works with clinical team to coordinate referral re-authorizations for home care and home health contracted agencies to avoid lapses in authorization or delays in services Monitors, tracks, and evaluates personal care services delivered to participants by contracted home care agencies to ensure services are aligned to care plans, including verifying completion of competency assessments Reviews and responds to home care concerns from participants and contracted agencies Attends the interdisciplinary team meetings as a member to communicate participant changes and participate in care planning Reviews and uploads home care and home health medical records to identify follow-ups; works with clinical teams to ensure follow-ups are processed in a timely manner Reschedules missed services or appointments and notifies clinical teams according to "No Show" policies Screens, answers, and responds to home care/health calls and messages EDUCATION/EXPERIENCE High school diploma or GED required; bachelor's degree in business administration or heath related field preferred Valid BLS-Basic Life Support certifications required on or before hire date One year experience working with frail or elderly population preferred Experience working in adult day health center or PACE preferred Bilingual (English/Spanish) required ADDITIONAL QUALIFICATIONS (Knowledge, Skills, and Abilities) Excellent verbal and written communication skills, including superior composition, typing and proofreading skills Ability to interpret a variety of instructions in written, oral, diagram, or schedule form Knowledgeable about and experience with adult day health centers and treating geriatric patients Ability to successfully manage multiple tasks simultaneously Ability to takes on call rotations on an alternating basis to include weekends and evenings Excellent planning and organizational ability Ability to work as part of a team as well as independently Ability to work with highly confidential information in a professional and ethical manner Physical Requirements Ability to lift/carry 50 lbs/weight Ability to stand for long periods of time Neighborhood Healthcare offers a generous benefit plan that includes: Partially company paid Medical, Dental, and Vision Plans.
