Home Based Primary Care Physician

The HBPC Physician position will encompass both administrative and clinical duties in Home Based Primary Care (HBPC). The Home Based Primary Care (HBPC) program is a long term care program which provides longitudinal, comprehensive, interdisciplinary primary care services in the home to Veterans with complex, chronic and disabling diseases

The administrative medical duties of the HBPC Physician are to direct, and be responsible for, the overall clinical care delivered by the HBPC team in the assigned sector. He/she will be expected to make home visits to patients receiving care in the HBPC program, carrying a panel of patients. The incumbent may precept medical students, residents, and fellows during home visits and will be expected to support of the teaching and clinical programs of the Medical Center. In addition, the HBPC Physician will also be expected to support other HBPC staff in the clinical management of patients whose primary care provider cannot be reached.

The direct clinical duties of the HBPC Physician will consist of management of their continuity primary care panel, including indirect activities of patient care: charting, returning telephone calls, reviewing and signing chart notes, reviewing and renewing prescriptions, and reviewing laboratory and imaging data. Clinical coverage of the other Geriatric and Extended Care programs (such as nursing home attending) will be expected with reciprocation from the Geriatric staff. The majority of the incumbent's work is expected to occur in the veteran's home setting but may also occur in other GEC venues for clinical coverage.

The clinical and administrative duties listed above are not comprehensive and may include other duties as determined by the program needs and assigned by the HBPC Medical Director.


Qualifications:

The incumbent must meet the VA professional and physical standards including citizenship, education, licensure, English language proficiency and references. Evidence of experience related to home care, interdisciplinary team care, medical directorship, and program development is required. Desirable attributes include: curriculum development, prior related research, and publications pertaining to general medicine and/or gerontology.


A general internist or family physician may be appointed to the Medical Staff of the VAMHCS Medical Center and assigned to the Geriatrics and Extended Care Clinical Center under the following conditions: 1) is a recipient of a degree of doctor of medicine 2) is an licensed independent practitioner who holds a current, full and unrestricted license to practice medicine in any state and 3) has completed at least, a three-year internal medicine residency and has successfully completed the Certifying Examination of the American Board of Internal Medicine or the American Board of Family Practice.


The incumbent must be an expert in history taking, physical examinations, test interpretation, patient counseling, differential diagnosis and medical therapeutics. He/she must also have sufficient experience in the evaluation and management of adults with single or multiple disorders in all of the subspecialties in the broad field of internal medicine. Prior experience providing care to patients in primary, secondary and tertiary care settings is necessary.
