Disease Intervention Specialist for the TB/Refugee Program - DOH-Pinellas Your Specific Responsibilities: The incumbent, a Health Services Representative, works in the Tuberculosis/Refugee Health Program under the direct supervision of the Health Services Supervisor in the Division of Disease Control and Health Protection in the Department of Health Pinellas County. The incumbent provides basic health and social services to clients in a variety of settings implementing effective case management practices. The incumbent performs field outreach activities working with individuals who are being managed and treated for tuberculosis disease and infection. The incumbent serves as a liason between licensed staff and the community in a culturally and linguistically appropriate fashion. The incumbent conducts self-according to the Core Performance Standards in the areas of Courtesy, Communication, Teamwork and Safety. Conducts home visits to ensure patient compliance with prescribed Tuberculosis (TB) medications by Directly Observed Therapy (DOT); Documents DOT on appropriate forms and logs; Follows established state and local rules, regulations, policies, procedures and guidelines for health care worker medication handling; adheres to local security procedures to keep medications safe for DOT performed outside the TB Clinic; Checks to determine that client medications match the medication orders and that the medication is given to the correct client; Observes patient for signs and symptoms of TB disease, adverse reactions and side effects to medications, non-compliant behavior, alcohol usage, and other pertinent information and immediately notifies the TB nurse if any are observed; Monitors patient/family for compliance with medical referrals provided by the TB nursing staff; Informs the TB nurse of needs or problems stated by the client during the term of their treatment; Assists individuals or families with issues that are crucial to their treatment and care; Makes decisions when to collect sputum specimen samples and obtains specimens; performs Mantoux Tuberculin Skin Testing (TST) and/or draws blood as necessary; Completes TB assessment/symptom screen when administering the TST or blood draw; Assists the TB Nurses in performing field investigations; Performs contact investigations locating tuberculosis suspects, cases and/or contacts for follow-up investigation and/or treatment. Provides clinical support on a rotating basis to include but is not limited to: scheduling appointments, providing education on Tuberculosis via phone, making sure staff have entered services, works in change healthcare, documents on note pad, runs daily service and coding reports, checks in clients, cancels appointments, assists and reviews all intake paerwork , scans all documents into client records, answers phone lines, refers clients to other clinics, provides TB and HIV screenings to clinic clients as needed, runs all monthly and daily reports, orders labs and monthly supplies. Documents all field activities in the HMS database following proper procedures while maintaining strict confidentiality at all times; Communicates TB related information and/or instructions to individuals and families in a culturally and linguistically appropriate manner; Monitors client clinic visits and schedules Direct Observed Therapy (DOT) and other appointments for the most efficient delivery of services. Assists with completion of Risk Assessment documentation; Submits completed EAR forms, time and attendance and other documentation as required. Performs other duties as assigned. Provides back up to other members of the Division of Disease Control and Health Protection as assigned to an operations level. This may include, but is not limited to: outreach efforts, immunization clinics, disease investigations and traveler monitoring.
