Position: Health Information Management (HIM) Document Router
Full-Time/Part-Time: Full-Time
Work Location: Hybrid position after completing required training period
Shift: Monday - Friday
Rate of Pay: $16.00 - $20.00 per hour

The Health Information Management (HIM) Document Router is responsible for routing electronic, mailed, and faxed medical documents in a compliant manner.
- The HIM Document Router indexes and routes a variety of e-faxed and/or N2N medical documents accurately into patient electronic health record in accordance with CNY Family Care's best practice and established policy and procedures to ensure daily fax holding tanks are worked daily and not back logged
- The HIM Document Router sorts, retrieves, scans and indexes a variety of paper medical records into patient electronic health records in accordance with CNY Family establish policies and procedures.
- The Health Information Management (HIM) Document Router links appropriate specialist visit notes in the patient electronic health record to open referrals and ensure referrals gets closed.
- The Document Router contacts short term rehab facilities to obtain short term rehab admission notes and discharge summaries as requested by CNY Family CareTransition Nurses and ensures documents are in the patient electronic health record by the time of the patient's transition of care office visit.
- Maintains strict confidentiality of health records in accordance with Federal, State law and CNY Family Care's established practice policies and procedures.
- Reviews CNY Family Care's outgoing medical record requests for completeness and ensures they are HIPAA Valid authorizations when needed; notes deficiencies and sends invalid authorizations back to patient with a letter explaining deficiencies.
- Performs regular quality checks to ensure all scanned documents are indexed according to CNY Family Care's established procedure and best practice.
- Responsible for cross training in all Health Information Management Department job duties, coordinating and following up on patient referrals, and pre-authorizations as requested by supervisor.
- Demonstrates professionalism, courtesy and a positive approach to communication, teamwork, and interaction with other team members, clinical staff and patients.
High school diploma or general education degree (GED) required; one to two years of college preferred.
Minimum of two years previous experience working in Health Care/ Medical Records required; or three or more years of related experience and/or training in a medical office which uses an Electronic Medical Record; or equivalent combination of education and experience.
