Health and Wellness Coach Job Details Job Location Cape Atlantic Youth ACT (Northfield location) - Northfield, NJ Position Type Full Time Education Level 4 Year Degree The Health & Wellness Coach assists the team in creating and implementing effective solutions to improve youth/family health and wellness. Provides individual and group support. The Assertive Community Treatment program serves youth ages 5-20 in Atlantic and Cape May Counties. The Program provides intensive and immediate access to treatment services and supports in youth and family's home and community. A multidisciplinary mobile team is tailored to each youth and their family and through a family centered approach will focus on crisis stabilization and assisting the youth with improving functioning and symptomology of mental health and emotional concerns. Interventions will assist with enhancing family functioning and overall stability in the family home.
