Job Summary

Job Duties:

Assures that all medication needs are met in accordance with proper procedures set forth by the policies and procedures for Group Homes.

Administrative

1. Shall train non-licensed staff who qualify for the training program after they have met the criteria established by the Department of Human Services.

2. Prior to training non-licensed staff, the Nurse Trainer shall perform the following for each client participating in self-medication programs and supervised by non-licensed staff only after they are trained and authorized:
  • An assessment of the client's physical and mental status and medical history
  • An evaluation of the medication orders and medications prescribed.
  • Document in the clients clinical record that such assessments and evaluations have been completed and the date of completion.

3. Shall establish initial competency-based training which shall include but not be limited to:
  • Name of medication or identification within the existing agency pharmacy protocol.
  • Dosage or quantity to be taken.
  • Route of administration.
  • Frequency or time(s) of administration.
  • Purpose of medication, special instructions, common side-effects and potential consequences of not taking the medication or of not taking the medication properly, and
  • When to seek medical assistance and any action to be taken in the event of a missed dose, medication error, or adverse drug reation.

4. Shall re-evaluate all authorized direct care staff at least annually or more frequently at the discretion of the RN Nurse Trainer.

5. Shall retrain to the extent that is necessary to ensure competency of such authorized staff if justification is apparent.

6. Shall retrain any direct care staff who fail to qualify as authorized direct care after initial training and testing within three months to be given a second opportunity.

7. Document the time of training, retraining, and evaluation of each staff in a separate employees file to be retrained in that location where the employee serves.

8. Approve a written protocol for each prn medication which shall include:
  • The name of the individual
  • The name, route, and dosage form of the medication
  • Dosage or quantity to be taken
  • Frequency of time(s) of administration
  • Conditions for which the medication may be given
  • A maximum or stop dosage
  • Any necessary special directions and precautions for the medication's preparation and administration
  • Common severe side or adverse effects or interactions and the action required if they occur; and
  • Proper storage

9. Shall review all medication error reports and assure all such reports are maintained. Assure that all medication errors are reported by phone within 24 hours and submit a written follow-up within seven days to region office of Illinois Department of Public Health and recommend any corrective action to be taken if deemed appropriate.

10. The RN Nurse Trainer shall in-service authorized staff of each medication change including change in medication dosage and time of administration and it shall be her responsibility to maintain records of each in-service. This shall occur prior to the next dose.

11. Shall review all drug reaction reports and proceed with further action if such action is deemed necessary.

12. The RN Nurse Trainer shall be on duty or on call at all times unless a conditional approval has been obtained from the DD Clinical Director to allow a registered nurse to monitor and direct the program for ninety days or less.

Qualifications

  • Licensed as a registered professional nurse in the State of Illinois.
  • Possess two years of clinical registered professional nursing experience within the last five years, at least one of which shall preferably have been in developmental disabilities.
  • Have successfully completed the Department of Human Service Nurse Trainer Program and received approval as a Nurse Trainer.
