SUMMARY OF DUTIES
The Gerontology Specialist provides case management support that includes completing assessments and coaching services to seniors who are enrolled in the State of Hawaii Veteran's Directed Care - Long Term Care Support Services (VDC-LTSS) and Community Living (CLP) Programs through the City and County. Services are provided to eligible persons who are at risk of long-term care placement or prevent possible institutionalization. The Gerontology Specialist also provides coaching support to help identify service needs to support participants and to alleviate burnout of caregivers.
EDUCATION AND TRAINING REQUIREMENTS
- Four-year College Degree from a school accredited by a recognized accrediting agency - general major. Requires basic knowledge or training in a specialized or technical field or trade, such as social work, professional accounting, finance, business administration, and marketing. Bachelor's Degree in Human Services preferred.
EXPERIENCE
- Over two years, up to and including four years.
SPECIAL KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
- Experience in working with functionally and cognitively impaired adults of all ages, elderly included, strongly preferred.
OTHER POSITION REQUIREMENTS
- This job requires a valid Hawaii driver's license, a clear driver's abstract, and use of personal vehicle to travel to participant homes to conduct in-person assessments and quarterly coaching visits.
- This position is subject to a criminal background investigation and continued employment is contingent on a record clear of convictions.
- Continued employment in this position is contingent on successful completion of CPR, and CPI training as such training is necessary to ensure ability/continued ability to perform essential functions of this job.
- Continued employment in this position is contingent on successful completion of First Aid classes.
