Job Title: Flexible Housing Pool Case Manager

Compensation (Based on experience and qualifications)

Location: South Chicago

Schedule: Monday - Friday 8:00am - 5:00pm

The Flexible Housing Pool (FHP)Case Manager will provide comprehensive, individualized support services using a client-centered harm reduction, trauma-informed model for clients in the Flexible Housing Pool. The successful candidate will be responsible for bi-monthly home visits to clients living in scattered site units across the Chicagoland area, accompanying clients to medical and benefits appointments, and connecting clients to community-based resources.

Responsibilities:
  • Provide comprehensive, individualized support services to clients in the Flexible Housing Pool
  • Conduct bi-monthly home visits to clients living in scattered site units across the Chicagoland area
  • Accompany clients to medical and benefits appointments
  • Connect clients to community-based resources
  • Act as a liaison between landlords/property managers and clients. Work with the landlords to resolve landlord/clients disputes.
  • Assist clients in meeting all lease requirements including documentation and income certification. Work with clients to assist them in addressing issues in order to prevent eviction.
  • Coordinate transition from bridge units, move-ins to permanent supportive housing and lease renewals.
  • Coordinate moving, furniture, and starter kit resources with the FHP team members.
  • Assist clients with rehousing should they require a move before their lease is up and/or at the time their lease is up. This includes completing housing lease renewal paperwork and submitting it to Flexible Housing Pool Payment Processing Department before deadlines to ensure continued housing stability.
  • Develop and maintain positive relationships with clients, community partners, and other stakeholders
  • Maintain accurate and up-to-date client records and documentation
  • Participate in team meetings and case conferences
  • Attend trainings and professional development opportunities as required

Qualifications:
  • Bachelor's degree in Social Services or related field and 1 or more years' Supportive Housing Case Management experience or three (3) or more years' Supportive Housing Case Management experience
  • Experience applying Homelessness and Supportive Housing concepts
  • Experience applying Harm Reduction and Housing First philosophies
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Strong organizational and time management skills
  • Valid driver's license and reliable transportation

Education and Experience
