Field RN Case Manager, Hospice (RN)

The Opportunity: We're actively seeking a compassionate and dedicated FULL-TIME Hospice RN for the Glendale area. This vital position involves providing exceptional hospice care within the warmth and comfort of our patients' homes throughout the valley. It's an opportunity to significantly impact the lives of patients and their families during their most tender moments.

Qualifications:

A graduate of an accredited school of professional nursing and licensed as a Registered Nurse in the state of Arizona.

A minimum of two (2) years of nursing experience is required, with at least one (1) year of specific experience in in-home hospice care.

Strong leadership capabilities, able to supervise and direct members of the client care team effectively.

Excellent written and verbal communication skills, with a knack for tactful interactions with clients and the community.

Demonstrated organizational skills and the ability to manage time effectively.

Must be a licensed driver with automobile insurance in good standing.

Typical Physical Demands and Working Conditions:

The role requires a full range of body motion, including handling and lifting patients, manual and finger dexterity, and eye-hand coordination.

Occasionally required to lift and carry items weighing up to 50 pounds or more.

Must have corrected vision and hearing to normal range, potentially work under stressful conditions, and work irregular hours.

Exposure to communicable diseases is common, given the nature of the work is predominantly in patient homes.
