Are you a Case Manager looking for that next step up the ladder? Do you have at least 2 years of case management experience and 2 years of supervisory experience? Have you worked in Human Services and/or with Developmental Disabilities before? If so, this may be the position for you. You will maintain a minimal caseload of individuals and families as well as provide general daily oversight and supervision of the Family Support group. In addition, you will:

  • Review Service Agreements, Amendments, and action plans.
  • Provide leadership to the Family Support staff in advocating and supporting families.
  • Serve as the liaison to the Family Support Council as well as local school systems for school transition planning.
  • Oversee the support and assistance provided to families per designated applicable NH regulations and waivers.
  • Implement and coordinate the Medicaid Respite Program.
  • Facilitate the legal processes of guardianship, Power of Attorney, and Advanced Directives.

Qualifications:

  • A Bachelor's degree is highly preferred though significant case management experience may substitute for a degree.
  • 2+ years of Human Services experience is a must with at least 1 year in Family Support. Experience with developmental disabilities is highly desired.
  • Prior supervisory skills are highly preferred.
  • Experience working with families from diverse backgrounds in a non-judgemental manner is a must.
  • Strong and effective verbal, interpersonal, and written communication is necessary as well as the ability to facilitate meetings.
  • Excellent organizational skills are required.
  • At least intermediate proficiency in MS Office and willingness to learn new programs is required.
  • Must complete our comprehensive background check process as well as pass a TB test.

This is a full-time, Claremont-based position. For this position, you will travel within our region of Sullivan and Grafton counties and other locations in NH as needed so a valid driver's license, proof of auto insurance, and reliable transportation are required (mileage is reimbursable). The position is initially on-site for training purposes and may transition to a hybrid work environment.

Compensation details: 23-26 Hourly Wage
