Your Role with the Company : Family Medicine Physician

  • The Family Physician will provide whole person healthcare from infants to older adults in an integrated care setting.
  • The Physician will diagnose and treat acute and chronic illnesses, provide routine health screenings and counseling on lifestyle changes in an effort to prevent illnesses before they develop.
  • The Physician will guide and coordinate all aspects of care as an engaged and active member of a multi-disciplinary treatment team.
  • The Physician will work with the team to achieve the best possible patient outcome in the most cost-effective manner.
  • Other staff include: Medical Director, Psychiatric Nurse Practitioners, Primary Care Nurse Practitioners, Medical Assistants, Patient Care Coordinators, Dietitian Nutritionist, front desk staff and behavioral health staff.
  • This position will serve 20 patients per 8 hour day and will provide supervision to Nurse Practitioner staff.
  • Rotating monthly on call, only on call once or twice per year with minimal call volume

  • Ability to hold a CT state medical license
