Join Benchmark as an Extended Services Case Coordinator to provide support to individuals in sustaining long-term employment following the closure of their vocational rehabilitation case.

Must be willing to travel the greater Fort Wayne area as well as the greater Marion area.

RESPONSIBILITIES:

  • Maintain accurate client records as per program and regulatory requirements.
  • Conduct regular meetings with individuals and employers on-site.
  • Offer support in navigating workplace dynamics and fostering relationships.
  • Facilitate re-referrals to vocational as necessary.
  • Provide outcome data to designated personnel.
  • Monitor and summarize monthly billing activities.
  • Perform periodic file reviews for accuracy and timeliness.
  • Coordinate all business between department and state funding bodies.
  • Oversee client programming and services.
  • Adhere to attendance requirements set by the program, Benchmark, and individual clients.

QUALIFICATIONS:

  • Bachelor's degree in a related field and/or one (1) year experience preferred with people with disabilities
  • Three (3) years experience of continued and verifiable employment within two years of submitting an application
  • Valid Indiana Driver's license and auto insurance
  • Reliable transportation
