A Discharge Planner is needed to work in an Inpatient Behavioral Health Hospital in Baton Rouge.
Listed are a few responsibilities but can be subject to change:
- Assists with planning appropriate level of care
- Work closely with physicians, nurses and clinicians to coordinate the patient's transition to a lower level of care
- Must be highly organized, empathetic and capable of multitasking
- Must participate in Staff and Team meetings to obtain collateral information to update the Patient Care Team placement options and other needs
- BS in social work Preferred
- 2-3 years of Psychiatric inpatient discharge EXPERIENCE IS A PLUS
