As a Director of Home Health, you will have the opportunity to connect with, and positively impact the lives of, our patients daily. You will promote the physical, personal, and emotional well-being of patients and work with the Dover Health teams to strive to deliver compassionate support in this critical time for patients and families. As a Director of Home Health, you will impact the lives of our patients by Coordinate and oversee all direct and indirect patient services provided by clinical organization personnel, ensuring quality and adherence to standards. Provide supervision, guidance, and appraisal of clinical staff performance to enhance service delivery and professional development. Perform chart audits, gather data for Quality Assessment and Performance Improvement (QAPI) activities, and participate in audits, Interdisciplinary Group (IDG) meetings, and QAPI initiatives. Identify and evaluate patient needs, assess the appropriateness of home care, and conduct home visits as needed. Oversee the maintenance of patient clinical records, ensuring compliance with local, state, and federal laws, as well as CHAP standards of care, and maintain adequate medical supplies. Assist with the orientation of new staff, provide in-services for current staff, and ensure compliance with Medicare, State, and CHAP regulations among all clinical staff. Monitor and implement strategies to meet Key Performance Indicators (KPIs) related to patient care quality and organizational goals and develop and track goals aimed at improving patient outcomes and operational efficiency. Collaborate with clinical staff to identify areas for improvement and implement evidence-based practices while fostering a culture of continuous improvement and accountability within the clinical team. Take responsibility for self-development by pursuing formal education, attending workshops and conferences, actively participating in professional organizations, and engaging in individual research and reading to improve Nursing/Management practice. To become a Director of Home Health, you will need Completion of an accredited School of Nursing program is required. State licensure as a registered Nurse is necessary. Minimum of 2 years of nursing experience in skilled home care is essential. Preference will be given to candidates with at least one year of supervisory experience. Demonstrated ability to effectively market services and maintain tactful interactions with customers and the community. Comprehensive understanding of business management principles, governmental regulations, and adherence to CHAP standards. Strong communication skills, both verbal and written, are necessary. Must exhibit self-confidence and maintain a positive attitude towards oneself and others while demonstrating unwavering commitment and enthusiasm towards achieving goals. Willingness and capability to travel as required. Additional responsibilities as delegated by management.
