Responsibilities Lead Value based care program planning, implementation and monitoring. Programs may be internally initiated or government, commercial, employer or community based. Evaluate possibility of ACO for the organization, determine financial risk versus opportunity for shared savings. Develop processes to decrease spend for Medicare beneficiaries, while improving health outcomes for those patients. Serve as liaison for commercial payor programs to improve the quality of care/health outcomes of patients with commercial insurances. Establish relationships with organizations within the community to focus on health/wellness for their employees to decrease spend. Work on Self-funded plans initially. Qualifications Bachelor degree in Healthcare Administration, Business Administration, Public Health, Social Work, or other related field required Five years experience in accountable care/population health program preferred One-year leadership experience required Regulatory and contractual requirement management experience Experience with analyzing and interpreting data
