As our Director of Care Management,now part of CommonSpirit Health formed between Catholic Health Initiatives (CHI) and Dignity Health, you'll be part of a leadership team that ensures our patient care experience is the best it can be. As a leader in care management, you will lead a team that works with physicians, other nursing teams, specialized departments, and various outside agencies to ensure that our patients are taken care of; well after they are discharged from our care. You'll build trust in these interactions by showing compassion, using your critical thinking skills, and remembering that each patient requires a custom care plan tailored to their needs. You will lead a team of professional care managers that support patients by identifying and removing patient care barriers that will regress their healing. Your talent for creative thinking is vital to your success in case management. It shines as you and your team develop enhanced discharge plans yielding optimal outcomes for each patient. Responsibilities Responsible for planning, development, organization, implementation and ongoing operations of the hospital care management department Provides leadership and direction to care coordinator staff members Ensures the coordination of services for a smooth continuum of care process throughout the organization Develops and implements standards of operation, policies & procedures, work flows and desktops to ensure quality management, fiscal accountability and outcomes measures Ensures compliance with regulatory agencies Qualifications Unencumbered RN license to practice in the State of Arkansas, or compact state Bachelors Degree required, preferably BSN Minimum of 5 years nursing leadership experience, preferably in an acute care setting Minimum of 2 years in a case management setting, preferably in an acute care setting CCM Certification preferred While you're busy impacting the healthcare industry, we'll take care of you with benefits that include health/dental/vision, FSA, matching retirement plans, paid vacation, adoption assistance, and more.
