SUMMARY:TheDirect Support Professionalprovides self-care training and assistance in daily living to individuals with developmental disabilities residing in a group home setting. ESSENTIAL DUTIES AND RESPONSIBILITIESinclude the following. Other duties may be assigned. + Must stay awake and not place themselves in a compromising position/place that is conducive to sleep; Continuously assess the status of the physical building and the individuals throughout the duration of the shift. + Provides individualized care as outlined in the individuals Life Plan, or ISP. + Supports the residents to ensure their safety and well being. + Observes, documents and communicates pertinent information such as behaviors, nursing, nutrition, clinical issues, etc., to facilitate assessment and development of treatment goals.
