Location BMH Bronson Methodist Hospital Title Diabetes Educator-Maternal Fetal Medicine - Part Time 24 hours/week The outpatient diabetes educator provides individualized diabetes self-management education/ training to individuals and groups according to The Scope of Practice, Standards of Practice, and Standards of Professional Performance for Diabetes Educators (AADE, 2011 update). Employees providing direct patient care must demonstrate competencies specific to the population served. Bachelors of Science required, BSN preferred. Two years clinical experience with focus in area of specialty desired. Teaching experience preferred Licensed Registered Nurse in good standing with the State of Michigan Obtain BLS Certification by the end of the orientation period All instructional staff will meet one of the following requirements: Certified Diabetes Care and Education Specialist (CDCES) or 15 hours of approved continuing education accrued annually based on the DSME program anniversary date. New instructional staff will have 15 hours of approved continuing education within 3 months of date of hire (includes contractors and paraprofessionals) All CEU's (program certificate and objectives) or CDCES certificate must be provided to program coordinator annually. Certified by the Certification Board for Diabetes Care and Education (CDCES) preferred. Must be willing to obtain certification within 2 years. (based on full-time status) Demonstrates a high degree of competency with computers, including the ability to use an Electronic Medical Record reporting system (training will be provided) Proficient in Microsoft Office Must be able to work independently with minimal supervision Must be able to effectively perform in a fast-paced, patients-first environment, and to organize, prioritize and problem-solve effectively Must possess excellent organizational skills and the ability to manage several projects simultaneously Work which produces high levels of mental/visual fatigue, e.g., interactive and repetitive or small detailed work requiring alertness and concentration for sustained periods of time, the operation of and full attention to a personal computer or CRT between 40 and 70 percent of the time. The job produces some physical demands. Typical of jobs that include regular walking, standing, stooping, bending, sitting, and some lifting of light weight objects. Provides quality diabetes self-management education based on client expectations and needs. Adheres to the American Nurses Association Scope and Standards of Practice. Complies with all federal and state regulations/standards (MDHHS, ADA, AADE). Responsible for staff, patient and community education programs, individual or group, as needed. Conducts a thorough individualized, age-specific, assessment. Develops an individualized interdisciplinary education plan with short and long term goals. Communicates, coordinates, monitors and documents the plan of care. Assesses the outcomes of educational intervention and revises the plan as appropriate. Uses age appropriate learning methods, appropriate behavioral/motivational strategies and reinforcement techniques. Demonstrates understanding of stages of behavior change and ability to counsel individuals on these stages through development of behavior change goals. Assesses patient readiness to learn and patient perceived needs.
• Facilitates educational/behavioral experiences that assist the patient in achieving identified goals. Updates educational materials to assure accurate, current and evidence-based information. Collaborates with a multidisciplinary team of health care professionals. Systematically evaluates the quality and effectiveness of practice and revises practice as needed. Participates in annual review of departmental outcomes. Actively involved in department CQI process. Develops and monitors clinical outcomes. Serves as a member of the Bronson Diabetes Advisory Committee. Participates in special projects, committee assignments and program development as needed. Assumes responsibility for professional development and continuing education to maintain professional competencies.
