Day Habilitation Specialist - Chenango County Norwich, NY (+Country+Club+Road+Norwich+NY+USA) Apply Job Type Full-time Description Day Habilitation Specialist Full Time, Monday-Friday, Daytime hours Wage: $17.50-$18.75/hr TheDay Habilitation Specialistwill be responsible for delivering services in one or more core rooms operated by the Day Habilitation Program. These duties and responsibilities include, but are not limited to: + Actively cooperating with the Program Manager and other DSP staff in order to achieve the goals and objectives of the Day Habilitation Program. + Responsible for the implementation of goal plans, data collection to monitor progress, and monthly progress notes. Goal Plans will be based upon the person's valued outcomes as identified in the person's ISP. + Responsible for all scheduling of community activities and transporting individuals to volunteer and recreational activities in the Community. Also provide the necessary supervision, training and support during these activities. + Direct supervision and periodic evaluation of the Day Habilitation Trainer. + Active consultation/communication with all professional staff to insure individual progress/safety and regulatory compliance. + Responsible for integration of individual program plans into the core room and overall program environment to insure efficient and habilitative service delivery. + Participate in meetings and regular staffing with the interdisciplinary treatment team for the purpose of Initial, Semi-Annual, Annual and Special reviews as required and/or determined by need, and to develop program strategies. + Responsible for complying with all waiver Regulations, Part 624 and 633 in total, and reporting violations, deficiencies, and/or incidents to the Direct Supervisor as they are discovered or indicated. + Responsible for addressing and/or reporting all observed or suspected situations which adversely impact on the health and safety of individuals attending program. + Responsible for the appearance and cleanliness of the assigned core room. + Responsible for maintaining and upgrading the core room and program environment to maximize habilitation, progress, and normalization. + Responsible for the development of individualized goal plans in accordance with the person's valued outcomes as identified in the ISP. + Responsible for maintaining accurate daily participant rewards, charts, logs and case/progress notes per the instruction of the Direct Supervisor and the individual treatment plan methodologies. Responsible for program wide responsibilities as assigned by the Direct Supervisor. These responsibilities could include, but are not limited to: + Ordering supplies for core room with approval from Direct Supervisor. + Lunch and/or Transportation duty. Feeding individuals. + Assisting with participant transportation. + Participating on program or agency committees. + Assisting with program projects, ie. Staff training, research, etc. + Covering other core rooms during staff shortages. + Changing participants and assisting in toileting or showering as appropriate. + Provide assistance during behavioral emergencies which include, but is not limited to counseling, verbal calming, supervision and/or physical intervention. Requirements + A Bachelor's Degree in Human Services, Special Education or a related field and previous experience working with the Developmentally Disabled. - OR - A combination of education and experience that meets the needs of the reviewing agencies. + A driver's license and driving record acceptable to the ageny's insurance carrier. + Candidate must be able to lift 50 pounds and not have any lifting restrictions which would prohibit him/her from assisting with toileting, transfers, physical intervention, lifting, etc. Salary Description $17.50-18.75
