As a Counselor at Third Street Family Health Services, you will play a crucial role in supporting the mental health and well-being of our community members. You will be at the forefront of crisis intervention and mental health support services. Your role will involve providing immediate counseling and crisis intervention to clients in distress, assessing their needs, and collaborating with our multidisciplinary team to ensure timely and appropriate care. The ideal candidate will possess strong clinical skills, a passion for helping others in crisis, and the ability to work effectively in a fast-paced environment.



Primary Function: Meets with patients, their families, guardians or agency representatives and completes behavioral health crisis and diagnostic assessments. Works to link individuals to appropriate levels of care for stabilization and assuring their safety.


Job Duties:

  • Conduct rapid assessments to determine the level of risk and create safety plans.
  • Provide crisis intervention and counseling services to individuals experiencing acute mental health crises.
  • Completes behavioral health diagnostic assessments.
  • Develop and implement individualized treatment plans based on assessment findings.
  • Deliver evidence-based therapeutic interventions to address crisis-related symptoms and concerns.
  • Creates positive relationship with patients, their families, guardians or agency representatives.
  • Observe and conform to the standards of Third Street Family Health Services.
  • Active collaboration with and supervision of other health care providers.
  • Collaborate with a multidisciplinary team of professionals, including psychiatrists, social workers, and case managers, to provide holistic care.
  • Advocates and refers individuals with serious mental illness and/or substance abuse/addiction to community agencies for treatment.
  • Networks with local social service agencies to remain knowledgeable about available services and the referral process for those services.
  • Maintenance of all required licensure, certification, registration, training, behavioral health knowledge (including but not limited to new or changing treatments and standards of care), and or expertise to meet the needs of patients of TSFHS as determined by the organization.
  • Acquisition of additional trainings, licensures, or certificates as deemed necessary by TSFHS in the ongoing determination and mission to meet the needs of patients and the community.
  • Maintaining on a current basis all appropriate patient records, and likewise to observe and conform to all laws, customs and ethics applicable to the behavioral health profession.
  • Practice, communicate, and embody the dispassionate and unbiased delivery of care regardless of personal bias or prejudice and as appropriate for a professional of the rank of LPC/LSW or LPCC/LISW at TSFHS.
  • Communicate with the Chief Medical Officer and COO on all matters relating to malpractice insurance status, including notification of any claims and full compliance with the risk management policies and procedures of TSFHS.
  • Participation in staff meetings and other meetings.
  • Participation in good faith in peer review audit of charts as designated by TSFHS.
  • Practice good stewardship of clinic and community resources by providing cost effective and efficient care to the patients in our charge.
  • Other duties as assigned.

Requirements:

Qualifications

State of Ohio LSW/LISW or LPC/LPCC license required.

Completion of a bachelor or master's level program

Prior experience in a community mental health agency, a plus

Strong clinical and crisis assessment and diagnostic skills.

Knowledge of evidence-based therapeutic interventions.

Excellent communication and interpersonal skills.

Ability to work independently and as part of a collaborative team.

Commitment to maintaining the highest ethical standards and confidentiality.

Cultural competence and the ability to work with a diverse client population.

Proficiency in electronic health record (EHR) systems is a plus.
