Summary: Focus on providing outreach and information to identified patient families in the Emergency Department on insurance coverage options, establishing a a primary care provider relationship, providing information on appropriate levels/sources of medical care, safety issues, and accessing community resources for unmet basic needs. Multiple patient families should be engaged, either in-person or by telephone, in these conversations during each shift. Responsibilities: Identify Medicaid recipients that utilizing the Emergency Department for non-acute care and discuss primary care physician resources, complete screening questionnaire with recipients to determine barriers in identifying or utilizing a primary care physician. Review barriers that contribute to the continued use of the emergency department instead of a primary care physician; complete follow-up on families to determine if a primary care physician has been identified and preventive care has been utilized. Provide families with community resources, as needed, and how to best access those resources. Discuss child safety needs (e.g., car seats, immunizations, etc.). How You'll Be Successful WORK EXPERIENCE At least 1 year job related experience Req At least 2 years job related experience Pref EDUCATION High school diploma or equivalent Req LICENSES AND CERTIFICATIONS Community Health Worker Certification Upon Hire Req NON-MANAGEMENT SKILLS Maintain effectiveness when experiencing major changes in work responsibilities or environment; adjust effectively to work within new work structures, processes, requirements, or cultures. Identify and understand issues, problems, and opportunities; compare data from different sources to draw conclusions; use effective approaches for choosing a course of action or developing appropriate solutions; take action that is consistent with available facts, constraints, and probable consequences.
