They represent typical elements considered necessary to successfully perform the job.Position DetailsAbout the Community Health Worker Apprentice: The 1-year apprenticeship offers 144 hours of didactic training, 2000 hours of on the job training, and has quarterly milestones. At the conclusion of the 1-year apprenticeship, the employee will be eligible to be certified as a Community Health Worker and transitioned from the Community Health Worker Apprentice to Community Health Worker.Hours: 9:30 a.m.- 6:00 p.m. (1 day a week) and 8:00 a.m.- 4:30 p.m. (4 days a week) Location and Travel: Community-Based position located in Pennsylvania. Position requires 80% travel for outreach to patients in Luzerne County. Use of company car after completing orientation.
