Community Health Specialist

Job Description Overview Supports Access to Reproductive Health Services and Contraceptive Care grant focused on increasing equitable access to reproductive health resources, including assessments and immediate access to contraception, for underserved women in Marion County through a coordinated continuum of care involving formalized referral partnerships and accessible patient navigation. Serves as a bridge between the community and the health care, government and social service systems. Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. Provides informal counseling, support, and follow-up. Serves as a bridge between the community and the health care, government and social service systems. Provides informal counseling, support, and follow-up. Must have experience working with patients in community settings. Bilingual preferred. Grant concludes: 10/31/25. High School Diploma or equivalent required. 3-5 years of experience required. Requires basic computer skills / Strong communication skills.
