Overview Pay range for this position is $30.36 - $45.38/hour. Actual pay is based on years of licensure. POSITION SUMMARY: The Clinical Support Nurse is responsible for processing all Home Care patient/provider calls and electronic message orders. This role is patient-centric and uses critical thinking skills that results in clinical excellence for home care. Analyzes patient needs, plans and prioritize care needs for patients of all ages. The Clinical Support Nurse is responsible for decision making, working closely with clinical staff, on-call nurses, and leaders to ensure the patient needs are met. The position will work as a resource for Team Members in the field and in the office. The Clinical Support Nurse may be required to utilize clinical judgement, assist with operational and patient care needs as they arise for all Home Care departments including Home Health and Hospice May live within any OSF locations. May need to travel for orientation purposes Qualifications REQUIRED QUALIFICATIONS: Licenses/Certifications: RN license by the State Board of Nursing in Illinois and/or Michigan, if not licensed in both IL & MI, additional Licensure (IL or MI) required within 90 days of hire Valid State Driver's License Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty. Other skills/knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem solving skills, with the ability to be detail oriented. PREFERRED QUALIFICATIONS: BSN Experience as an RN in Home Health or Hospice Other requirements/information: Electronic Medical Record and Microsoft application basic proficiency Customer Service Must have professional verbal and written communication skills. Travel to appropriate agency when needed Analytical and critical thinking skills in order to manage issues/concerns of patients, and make decisions in order to ensure patient needs are cared for.
