The ALS RN Coordinator is responsible for monitoring medical, physical, and psychosocial care needs of patients receiving care from the Providence Amyotrophic Lateral Sclerosis (ALS) Center. The RN provides direct patient care and case coordination from diagnosis to end of life. This position coordinates the care of the patient through assessment, planning, implementation and evaluation of appropriate services and resources to maximize optimum outcomes. Works collaboratively with the multiple disciplines, both internal and external to the organization, in effecting quality and cost-effective management of the patient's care along the continuum of chronic neurologic disease. Required Qualifications: Bachelor's Degree in Nursing; from an accredited RN School of Nursing. Upon hire: National Provider BLS - American Heart Association Upon hire: Oregon Registered Nurse License 2 years Nursing experience working with patients who have neurological conditions or ambulatory care, home health, acute care and/or case management. Background in telephone triage, outpatient clinic, ambulatory care, home health, acute care, hospice, and/or case management. Experience with an electronic medical record system.
