This position is located at Sequoia Regional Cancer Center Qualifications: License /Certification Required: Current California Registered Pharmacist license Preferred: Pharmacy Practice Residency Certificate Education: Preferred: Pharm. D. degree Experience: Preferred: Equivalent hospital/outpatient pharmacy experience Department Specific Requirements If assigned to Outpatient Oncology Infusion Center Doctor of Pharmacy (Pharm. D.) degree required. Completion of PGY 1 residency or at least two years of acute inpatient hospital pharmacist experience. Prefer completion of pharmacy residency in Oncology Addendum (essential for specific dept) OUTPATIENT ONCOLOGY PHARMACIST - OUTPATIENT ONCOLOGY INFUSION CENTER Serves as the clinical pharmacy expert in the area of outpatient oncology services, integrating advanced knowledge and training to ensure desired service and patient outcomes are achieved. This dedicated pharmacist resource will collaborate with pharmacy leadership to ensure safe and efficient compounding, preparing and dispensing of oncology therapies in the outpatient oncology infusion center Assists pharmacy leadership in organizing and directing daily pharmacy operations and pharmacy personnel, including training, education, and competency completion to ensure regulatory requirements are met Assists pharmacy leadership in maintaining records for pharmacy licenses, staff licenses, certifications and competencies as appropriate Participates in coordination of principal pharmacy related department operations, including committee functions participating in medical staff and organizational related committee meetings as they relate to pharmacy services in outpatient oncology. Interacts with other department leadership to solve mutual problems Guides, directs, evaluates, and trains pharmacy personnel and assumes responsibility for quantity and quality of their work Completes staff scheduling as it relates to pharmacy services Assists Manager with performance evaluations for assigned pharmacy employees Assists in drug inventory management and provides periodic performance activity reports to Pharmacy Director and assists in budget oversight as it relates to pharmacy services and in collaboration with clinic leadership Establishes annual goals with manager and is able to successfully show progressive achievement Oversees compounded preparations within the outpatient oncology infusion center pharmacy and collaborates with key stakeholders to ensure quality of compounded preparations and compliance with regulatory and best practices involving compounding Participates in regulatory body surveys, fielding questions and matters related to compounding , and other pharmacy related operations and services Develop, implement and maintain pharmacy related oncology infusion center related policy, procedures, order sets, guidelines & operational workflows to ensure quality, safety, compliance, and operational performance. Reviews and revises pharmacy related policies and procedures at least once a year WHEN ASSIGNED TO WORK AS A CLINICAL PHARMACIST: Dispenses medication, supplies or equipment orders accurately and efficiently. Follows written policies and procedures regarding medication administration. Consults with other health care providers and information resources to optimize therapy and maximize safety for medication administration for assigned areas. Uses PYXIS units on the nursing units. Responsible for answering all inquiries from physicians and other staff regarding medication administration issues.
