Day-to-Day tasks include, but are not limited to:

  • Maintain relationships in the Children's Hospitals and outside agencies
  • Receive and assess all referrals and disseminate information to the clinical team
  • Meet with families to educate family on Pediatric Specialty Care services and provide family a tour of the facility
  • Coordinate admissions by coordinating care with pediatric hospitals, case managers and social workers to ensure smooth transition from the hospital to Pediatric Specialty Care
  • Track Pediatric Specialty Care residents during their hospitalizations to provide continuity of care upon the patients' return to the facility
  • Communicate with physicians/leadership of facilities regarding status changes of the children
  • Attend Pediatric Conferences to educate community on PSC services and to market to the community



  • Current PA RN license/ preferably BSN
  • 2 years Pediatric experience preferred
  • Valid driver's license
  • Ability to travel to various locations on a weekly basis
  • Good communication skills
  • Must be 21 years or older

  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
