Nye Health Services is seeking an experienced clinical professional to join our home office team as Clinical Director of Admissions. This centralized role will lead and support the admissions process for all Nye skilled nursing facilities in Fremont and Lincoln, NE. As Clinical Director of Admissions, you will serve as an advocate for residents during the pre-admission process, work with all clinical teams to evaluate potential residents' medical needs before admission, serve as a resource for referring hospitals by developing relationships with their physicians and case managers to achieve census goals and identify potential residents that will benefit from Nye Health Services quality care and services. This centralized home office role will offer flexibility in work location including the possibility of working remotely. Candidates should be willing to live within a reasonable driving distance of Fremont or Lincoln, NE as routine onsite visits will be necessary. Essential Duties and Responsibilities Armed with a strong clinical background, payer knowledge and problem-solving skills, you'll work with physicians, discharge planners and patient's families to plan the best continuum of care. Manage all aspects of the nursing pre-admission process including assessment for appropriateness of admission, coverage, and level of care determination. Perform clinical assessments of referred patients, then counsel with patient's family to assist them in making the best care decisions for their loved one. Partner with hospital case managers to create smooth patient transfer processes and help remove payer-related and operational barriers to getting appropriate patients into this highly effective level of care. Coordinate with campus admissions teams and work closely with all disciplines to prepare them for admissions and their special needs. Proactively work with a multidisciplinary team to manage care, costs, and discharge planning. Manage daily census numbers, including admissions and discharges. Education & Experience Registered Nurse or Licensed Professional Nurse with valid current licensure with the State Board of Nursing in Nebraska is preferred but not required. Previous admissions, intake, case management, or liaison experience preferred. Long-term care, acute care, or subacute experience, preferred. Basic knowledge of MDS, payer systems, and contracts (Medicare, HMOs, Managed Care, Third Party Payer Systems). Demonstrated organizational skills and abilities to handle multiple priorities accurately and on time to facilitate customer and program needs. Ability to handle inquiries and relate well with referral sources, prospective residents, and their families, and to handle confidential information and admission records. Ability to communicate effectively and appropriately in a wide variety of circumstances, both verbally and in writing. Be an investigator and problem solver - use your critical thinking skills to overcome obstacles that prevent patients from being admitted to the optimal level of care.
