Candidate Requirements

• State License(s) accepted: MN
• DEA required: Yes
• Covid/Flu Vaccination Requirements: Required, exemptions may be filed
• BC/BE: May be board certified or board eligible
• Boards accepted: FM or FP
• Residents: Y/N and what are the details?
• Life Certifications needed: BLS, ACLS
• Will facility accept CALS in lieu of ATLS: N/A
• Procedures logs needed: N/A
• Other qualifications needed: N/A

Current Staffing/Needs/ of hours/shifts
Schedule for Hospital would be on call one clinic day from and one weekend a month. During one clinic/call day per week Physician would round in the morning and then start clinic for the day. Weekends no clinic but would round on patients and on call from
